Sutter Tracy is committed to providing employees with medical coverage designed to promote healthy living, and in the event of injury or illness, protect you and your family. Current insurance plans include medical, dental, vision life and long term disability.
If you elect not to participate in Sutter Tracy’s health insurance programs and can provide proof of other insurance, you may opt to receive a benefit reimbursement.
Eligibility & Changes
Full-time employees (minimum 72 hours per pay period) and part-time employees (minimum 40 hours per pay period) become eligible for benefits on the first day of the month following 90 days of employment. At this time your eligible dependents may also begin to receive coverage under some of the plans.
Once you have enrolled in Sutter Tracy benefit plans, changes may be made during our open enrollment period in November of each year, or when you experience a change in family status such as marriage, birth of a child, or change in employment status of you or your spouse.
Medical
We offer two Pacificare HMO plans from which to choose. Under the low option, Sutter Tracy covers your individual premium in full. Your children and spouse may be added for $108-140 per month. The office visit co-payment is $15, with unlimited benefits.
Under the high option, the employee pays a small fee of $38 per month, and the rate to add your children and spouse is $190-256 per month. Under this option the office visit co-payment is only $5.
Both options include unlimited benefits, prescription drug benefits, a $50 co-payment for emergency or urgent care, and an annual co-payment maximum of $2,000 (3 individual maximum per family).
Dental
We also offer a low and a high option for dental coverage, both through Guardian. Under the low option, Sutter Tracy covers your individual premium in full, and you may add your children and spouse for approximately $30 per month. The annual deductible is $50 per individual, or $150 per family, and the plan provides 80% coverage on preventive care, and 80% coverage on basic and 50% on major services, with a maximum annual benefit of $1000 per member. There is no orthodontic coverage under this option.
The high option can be selected for $12 per month for the employee, and $32-40 per month with children and/or spouse. This option provides greater benefits for in-network dentists, with an annual deductible of $50 per individual or $150 per family. Under this option, diagnostic and preventive services are covered at 100%. Basic/major services are covered at 90%/60% if using an in-network dentist, or 80%/50% if using an out-of-network dentist. This plan covers orthodontics at 50% up to $2,000 per year.
Vision
Eye care is a critical part of your family’s health. Vision Service Plan (VSP) provides benefits for a wide range of doctors throughout Northern and Central California. When visiting a VSP network provider, the annual deductible is only $25 and you will be fully covered for examination and lenses once per year, and frames of your choice up to $120, plus 20% off out-of-pocket expenses. Contact lenses are covered every 12 months in lieu of lenses and frames. This plan also includes extra savings and discounts such as Laser vision surgery discounts, up to 20% savings on lens extras such as scratch resistant or anti-reflective coatings, and exclusive pricing on popular brands of contact lens supplies.
Life Insurance & Disability
Group Life and AD&D
Sutter Tracy provides the following life and AD&D coverage at no cost to our employees.
• Full-time employees receive $50,000 and part-time employees receive $25,000.
You may also opt to increase your life insurance coverage and buy coverage for your spouse or children up to age 19, or up to age 25 if the child is a full-time student. If you elect to purchase supplemental life insurance, the additional premium will be deducted from your paychecks depending upon the amount purchased.
Long-term Disability
Sutter Tracy provides the following LTD coverage at no cost to our employees.
• 50% of earnings up to $1500 per month.
Employees may elect to purchase additional coverage for the amount of 60% of pay, up to a maximum of $3500 per month.
Paid Time Off (PTO)
Sutter Tracy is proud to offer the best PTO program in the area. Full-time employees (72-80 hours per pay period) earn PTO for vacation, plus 10 holidays and 5 sick days per year. Part-time employees (40-71 hours per pay period) earn PTO for vacation plus 3 holidays and 2 sick days per year. Vacation accrual rates depend upon length of service and number of hours worked.
Extended Sick Time
Full-time employees who are ill for three consecutive days or more can also take advantage of extended sick without having to use their PTO, with a doctor’s note. Extended sick time can be utilized beginning on the third day of illness, up to 56 hours per year.
Retirement Benefits
Sutter Health Retirement Plan
All employees are automatically enrolled in the Sutter Health Retirement Plan at their date of hire. A percentage of your pay is added to your account annually, based on years of service with Sutter. The longer you work, the greater percent of pay you receive, and you earn a guaranteed rate of interest on you balance. Your benefits are 100% vested after three years of service within the Sutter Health Network.
Tax Deferred Savings [403(b)] with Employer Match
A 403(b) program is available to all employees. You may choose the percentage of pre-tax pay you would like to contribute (1%-20%), up to a maximum dollar amount per year as indicated by the IRS. Your funds are invested with VALIC, which offers a wide range of investment options.
In addition, after your first year of service (1,000 hours minimum) Sutter Tracy will match your VALIC contributions at 50% of the first 6% of your pay (maximum 3% of total annual compensation, as defined within the plan document). Employer contributions vest at a rate of 20% per year, and are 100% vested after completing five years of service.
Early Retiree Medical Plan
Employees with at least ten years of service who retire at age 55 may continue to receive the same medical benefits as active employees. While Sutter no longer covers your monthly premiums, you will continue to receive the negotiated group rate for medical benefits – a significant savings over individual plans.
